Alumni Immunization Records

Health Services keeps immunization records for 10 years from your date of graduation. To request a copy of your records please fax (508-793-3610) a letter indicating the following:

• Name
• DOB
• Year of graduation
• Fax # or address of where you would like to have records sent
• Phone number where you can be reached
• Signature
• Date of request

Your request will be sent within a week. You may call us at 508-793-2276 with any questions.
